Hello! I'm Ms. Lopez, the Registrar. Once you complete this form and submit it, it'll go directly to me. You may list multiple schools. If it cannot be sent via TRex, (our electronic system), your official transcript will be mailed to the school you are requesting it to go to. Please note that if you are requesting a transcript for a scholarship or a Military recruiter, those will have to be picked up at the school by you. Also note that with the amount of transcript requests that are received, I WILL NOT be sending out any confirmation emails for all requests.
