If the recipient must be an individual's name (ie. name of conductor), please provide the individual's SIN or business number for their required T4A. Choral Canada is required by law to create T4As for cash prizes.

This type of membership is acquired via the choir’s membership with their provincial choral organization, not acquired directly through Choral Canada. In provinces or territories where there is no choral organization, choirs must be a member of the nearest or most appropriate provincial choral organization.
