SLALN Intent to Participate Form
This form is your registration for SLALN programming for the 2019-2020 school year.

Planning these activities requires knowing the number of students who will attend. Your commitment to these activities by 10-1-19 is necessary to ensure that proper planning occurs.

Payment of all dues and fees is expected by October 31st, 2019 to be able to participate in programs for the 2019-2020 school year.

Please note, payments can not be refunded if you cannot attend a program you have registered for.

Math 24 - Fifth Grade - Double Digits: Team of three plus one proctor per team, Meets held February and March, $25 PER TEAM *
Math 24 - Sixth Grade - Exponents and Integers: Team of three plus one proctor per team, Meets held February and March, $25 PER TEAM *
Math 24 - Seventh Grade - Fractions and Decimals: Team of three plus one proctor per team, Meets held February and March, $25 PER TEAM *
Math Meet Fifth/Sixth Grade (1/14/20, snow date 1/15/20): Team of 4 fifth graders & 4 sixth graders, both genders. 2 adults minimum per team, lunch on your own. UW-Whitewater. *
Math Meet Seventh/Eighth Grade (1/14/20, snow date 1/15/20): Team of 4 seventh graders & 4 eighth graders, both genders. 2 adults minimum per team, lunch on your own. UW-Whitewater *
Touch of Red: HS art students display artwork and share their future goals at Southern Lakes Anthology Ceremony on 5/19/20. Fee is for district. *
Science Immersion Day: 10/29/19: MSOE (spaces filled for 2019, only check yes if already registered): Team of 4 fifth/sixth graders plus one participating adult. Sack lunch. *
Academic Bowl: Spring Dates: 15 students, 2-3 participating adults (5 sixth graders, 5 seventh graders, 5 eighth graders) *
Sweet 2B Me: Social/Emotional Day covering topics such as executive function, perfectionism, etc. $5 per child, bring your own lunch. 5th/6th: 10/10/19; 7th/8th: 10/8/19. Please designate approximate number of students (at $5 each) 2 students *
