If you are potentially interested in additional projects, please add a brief explanation and a list of project titles under "project-specific questions" below.

Upon completion of this form, faculty research project supervisors may contact you via email so that you may optionally provide additional information on classes taken, prior research experience, etc. Please redact perm number, telephone numbers, and other personal information from grade transcripts and other documents before sharing.
