Private Swim Lessons Form Fairwood Golf & CC - 2025 Season

Please fill out this form to get your Child's private swim lessons scheduled. You will work out a time individually with your assigned instructor. Our certified lifeguards will be in contact within 72 hours of form being completed to schedule your lessons (Starting In June). Lessons are for any swimmer above the age of 3! Anyone 3 or older (including adults) can sing for lessons. Please do this from for every individual swimmer. If you have two swimmers, please do this form twice (and so on). Beginner 1 - Never swam before or new to water

Beginner 2 - Beginner and is okay with being in water

Beginner 3 - Has experience and/or taken lessons before

Beginner 4 - technique based, knows strokes, can swim whole length

Intermediate - Knows how to swim, possibly on swim team, looking for dive lessons, starts and turns, and or more intence swimming

Advanced - On swim team, 12+ Only, Looking for one-on-one practice with Swim Coach, and a more advanced lesson
