Please use this form to sign up your children for the Summer Reading Program at the library.
Fridays at 10:00AM outside of the Menominee County's Library in Stephenson, or next door in the Stephenson Township Hall in inclement weather.

Dates: June 23, June 30, July 7, NO SUMMER READING JULY 14 - FAIR WEEKEND, July 21, July 28, August 4.

This is the permission slip and media release for the program.

We ask that all children are accompanied by a responsible adult.
Either a paper form or online form are required, not both.

This year, for our first week, we will be making tie dye t-shirts!! Would you like to provide your own t-shirt or have the library provide them? *

We will supply our own t-shirts (a cotton tshirt, white works best, any light color would also work)

We would like the library to supply our t-shirts

Names, Ages, and T-SHIRT SIZE (if you would like the library to provide your child a t-shirt) of Children Attending *

I, by submitting this form, do hereby grant or deny permission to the Menominee County Library to use the image, voice, and/or video of my child. Such use includes the display, distribution, publication, transmission, or otherwise use the images and or video of my child for use in materials that include but not limited to printed materials such as newsletters or newspaper, videos, and digital images such as those on Facebook. I understand my child’s image will never be sold or misused. *
