2017 Summer Music Camp Registration
Thank you for your interest in our Summer Music Camp. Registration is a simple, two-step process that will only take you a few minutes. The tuition for each session is listed below, and includes a keepsake DVD of the final showcase performances.

Indemnity: By checking the box below labeled “I agree”, I, the parent/guardian of the child I am registering for the Summer Music Camp being conducted by THE EVERGREEN CHORALE, a Colorado non-profit corporation,(“EC”) hereby state and agree as follows: 1. EC is offering a Summer Music Camp (“Camp”) for the purpose of furthering children’s education and experience; 2. Child has elected to participate in such Camp, 3. Parent or guardian, in consideration of the minor being afforded the opportunity to receive any educational and musical benefits which may be realized from said camp, hereby agree with EC as follows: 4. Parent or guardian agrees that Child participate in such Camp and authorize EC to involve Child in such camp and agree that Child will be under the control and authority of the EC and that EC may exercise parental supervision over Child during Camp activities, including emergency medical or surgical treatment when parents/guardians cannot immediately be contacted. 5. Parent/Guardian agrees to indemnify EC and its Directors, employees, and agents, both individually and in their official capacity, from and against all claims, suits, damages, costs, losses and expenses in any manner resulting from or arising out of Child’s participation in such Camp. Any medical, hospital and surgical expenses incurred as a result of such emergency treatment shall be borne by Parent/Guardian and, to the extent that such expenses may first be paid by EC, Parent/Guardian agrees to reimburse it for such expenditures. THIS AGREEMENT SHALL BE BINDING ON CHILD, PARENT, THE EVERGREEN CHORALE, AND THEIR SUCCESSORS.

Cancellation: If you have to cancel your child’s participation, you can receive a 100% refund if we hear from you prior to May 15th. A 50% refund is available after this time. There is a minimum number of students required for each session to run. If a session does not meet the minimum required, refunds will be made in full. There will be a $25 charge for all returned checks. You will receive an initial confirmation once we receive your payment.

Late Registration Fee: There will be a $20 fee added to all registrations after May 26 - so register early!
