New Staff Computer Account Request
This form is to be used by new staff to request a school login and computer account. This account will provide access to school computers, to the School Tool student management system, and will give the staff member Outlook and Gmail email accounts where appropriate. At the end of this form are links to the AUP policies for staff and students. New staff must read and acknowledge both policies in order to obtain access to the school's data and network.
