The purpose of this form is to report an incident, or concern, regarding any form of sexual misconduct, discrimination or harassment in the Boise School District. Completion of this form does not constitute an open investigation. Once this form is completed and has been received, an initial assessment will be conducted to determine if the report rises to a level that warrants further investigation.
This form is not an emergency incident report. If you feel threatened in any way, or have concerns about an immediate need, please contact your building administrator or call the Boise School District Office at 208-854-4000.
