Please note that this form does not guarantee the dates or details of your rental; a member of our management team will review all provided information and contact you for confirmation and deposit payment within 3 business days.

This form must be completed 4 weeks prior to rental date.

A detailed pricing outline is included in the second section of this form. Rental fees range from $250-$1000 based on number of guests, but please note that this fee does not include licensing, which is required for most film screenings. Our Midtown Cinema staff is happy to help guide and facilitate the licensing process as part of your rental.
