This form must be completed by every parish in the Reformed Episcopal Church regardless of whether any lay representatives or clergy attend General Council or not. This information is vital to providing an historical record of our activities as well as helping the Denominational office provide accurate information in the directory and website.
There are three (3) sections required. You will need to know the basic information regarding the parish (address, website, etc.) You will also need to know the names of the lay members elected by the parish to represent the parish at General Council (and, for Mid-America parishes, the names of those elected for Synod). Finally, you will need the following general membership statistics and official acts information (totals not names). These should be readily available to you if you report these each year at the Annual Parish Meeting.
- Baptized Membership (as of 12/31/2016)- Communicant Membership (as of 12/31/2016)- Current Average Sunday Attendance- Total Baptisms during years 2014, 2015 & 2016- Total Confirmations during years 2014, 2015 & 2016- Total Transfers into the parish during 2014, 2015 & 2016- Total Inactive/not-in-good-standing members restored to active membership during years 2014, 2015, 2016- Total Transfers out of the parish during years 2014, 2015, 2016- Total deaths of members during years 2014, 2015, 2016- Total active members moved to inactive/not-in-good-standing membership during years 2014, 2015, 2016- Total marriages officiated by clergy of the parish during years 2014, 2015, 2016
