Welcome to the Oregon Coalition Against Domestic and Sexual Violence’s online community assessment. We are collecting responses to this survey through October 31, 2017.

This survey will take approximately 20 minutes to complete.

Your answers will inform a to-be-published report on the status of domestic violence and sexual assault survivors and services in our state. Please stay tuned to ocadsv.org for more information.

Content warning: Please note that if you are a survivor of domestic violence or sexual assault, this survey may ask questions about the services you have received. It will not ask questions about the types of violence you have experienced.

If you are unable to complete the survey for any reason, you may exit the survey by closing your browser window at any time. Your answers will not be recorded unless you submit the form at the end of the survey.

If you need support at any time, please contact Call to Safety at 1-888-235-5333 to speak with a trained domestic violence and/or sexual assault advocate.

If you have any questions about this survey, please email Rebecca at rebecca@ocadsv.org.
