Parents and Guardians need to give permissions for: student(s) to participate in off-campus activities;  student(s) use of varied transportation modes in support of fieldtrips and off campus activities; use of student(s) in photos, videos and/or voice recordings from school programs and activities on website or in other school communications.

The attached form must be completed and returned before the student will be permitted to participate in any off campus activities.
