Wellness Initiative Submission Form
Statement of Need:
The health of our nation is not solely defined as the health of our population. In today’s world, we must address what impact our habits have on the health of our food system.

To create a resilient, healthy society, we need to consider the connection between our health and the environment (built and natural). Creating a more sustainable approach to our food system as a whole would not only ensure better health outcomes for people, but would reduce our growing chronic disease burden, minimize the impact on the environment, and distribute healthy, nutritious food in an equitable and fair manner to communities across the nation.

We believe innovative and progressive wellness and nutrition initiatives that utilize a food systems approach are able to have a larger impact on issues, specifically as they relate to food, health, and the environment. This can include but is not limited to work that connects nutrition and health with agriculture, family wellness, community development, policy, research, and education.

With that in mind, please fill out the form below.

Submission Information:
Thank you for taking the time to fill out the form below. You are being asked to submit further information because we believe you, the group you work for, or the members of your community have a nutrition or wellness program or initiative you would like to have highlighted within our report. We are asking that you submit information on the project or initiative you are currently working on or have previously implemented within your organization or community to The Harkin Institute for Public Policy and Citizen Engagement. The information you provide will be used to select the programs and initiatives we plan to highlight this coming year.

At The Harkin Institute, our goal is to facilitate collaborative, high-quality, nonpartisan, multi-disciplinary public policy research and analysis in the area of wellness and nutrition. As part of this goal, we are in the process of creating a report that will highlight various wellness and nutrition initiatives and programs throughout the country that strategically connect food systems with health and wellness. We plan to share this report with people, communities, businesses, and legislators with the hope that these programs will help other groups implement their own wellness and nutrition programs within their communities and organizations.

Please submit your response by Wednesday, November 7th, 2018. If you have any questions contact Lyndi Buckingham-Schutt (lyndi.buckingham-schutt@drake.edu).
