DISCLAIMER STATEMENT We/I accept that Christ Church Balham will not be liable for any loss, damage, action, claim, costs or expenses suffered as a result of my child/children attending Dangerous Club and that all children attending Dangerous Club do so at their own risk.
If you have any questions or would like more information on the precautions we have in place for particular activities then please do get in touch and we will be happy to chat to you and send you the relevant risk assessments.
Dangerous Club is run by Christ Church Balham, a part of The Co-Mission Churches Trust (Company no. 07496944 and Registered Charity no. 1139922), which is the data controller for the personal data which you have provided about you and your child.
We will use the personal data which you have provided in this form (including any medical data relating to your child) for us to plan and run Dangerous Club, specifically in relation to your child's attendance of Dangerous Club, and to ensure their safety and security whilst at Dangerous Club.
We will only contact you in case of an emergency, and occasionally with important information regarding Dangerous Club or events related to Dangerous Club.
We may also from time to time anonymise the data we hold on your child, for example to produce generic statistics about the number of children who attend Dangerous Club over a given year.
The information provided in this form will be stored as a password-protected completed Google form, until the information is transferred to a password-protected spreadsheet. Only the leaders in charge of Dangerous Club will have access to the Google account where the completed forms are stored, and to the password-protected spreadsheet.
The personal data which you provide will be held by us for the duration of the academic year in which it is provided, following which we will review on a regular basis whether we need to continue to hold this data. Factors relevant to this review will include: (i) whether your child is still in the Dangerous Club age range (school years 3 to 6); and (ii) whether your child is still regularly attending Dangerous Club.
You have the right to withdraw your consent to the processing of your and your child’s data. However, if you do so, unfortunately your child will be unable to continue to attend Dangerous Club, as this data is required for us to keep them safe. Please contact us (email@example.com) if you wish to withdraw your consent, or if you have any other queries about how we hold your data.
We will seek to keep the personal data which you have provided up to date, and will usually request that a new form is filled out for each academic year to ensure that this is done.
