This form is to register the attendance of yourself, or a youth member, at an Event (including Training).
It needs to be filled in for each individual attending the Event for the purpose of Health & Safety.
(Note: Piopio Adventure Camp and Summer Camp registrations use separate registration pages, and do not use the registration form below. Cossgrove and Sandford youth/leader-support registrations are also done by a separate process.)
