My Child will be attending the Marching Band Trip to Syracuse, NY on October 27–October 30, 2017. I realize that the payment of $205 including dry-cleaning ($20) for the season is due on August 14, 2017. Checks can be made payable to Midland Park High School.
I give permission for funds from my Child's Performing Arts Parents Student Account to be used toward the cost of this trip
I understand that my child will be required after July 1, 2017 to submit Medical Forms appropriate for the trip as described in an email that I will receive from Mr. Marks in June. Failure to submit such forms may cause my child to be removed from the trip. All Required Medical Forms are to be turned in no later than 10/2/17
