This form is for a teacher or parent of a current GSD student requesting:
     A novel be removed from the approved list
     A review of a previously nominated but not adopted novel
     A change in approved grade level
     A change in  a novel's annotation

Please be advised that you are expected to read the entire novel with the intended student audience in mind.
