Photo Release and Annual Membership Fee
(1) Participants in Amateur Youth Sports Foundation (AYSF) events are sometimes photographed and videotaped for use in promotional and educational materials. I understand that, if I provide consent herein, such audio, video, film and/or print images of Participant student may be edited, duplicated, distributed, reproduced, broadcast, and/or reformatted in any form and manner without payment of fees. I authorize AYSF to record and photograph Participant student’s image for promotional purposes.
(2) In order to maintain my AYSF membership and be eligible for any future volunteering works & awards, I will submit an annual membership fee of $50 to either Arizona or California Division office. A membership fee is non-refundable.

Q: Can I pay my membership fee with personal check?
A: Yes, please make your check payable to "Amateur Youth Sports Foundation" or "AYSF." You can either mail or submit your membership fee to any regional office.
