Welcome to Castaic's Wellness Center Request Form.

If you are stressed, overwhelmed, concerned and/or dealing with something personal, we are here for you.  Please reach out and schedule a private appointment by completing this form.

We are here for students, parents and staff. We realize we ALL need support at various times in our life and we hope to support you to find ways to recognize your strengths, find solutions to your struggles, find healthy coping mechanisms to help you help yourself or at least find a support person you can vent to. We never want someone to feel like they have no support or help.

Schedule an appointment here or feel free to stop by the center during school days (Monday through Friday 8am-3pm). If this is an emergency, please contact 911 immediately.
