MidwestRP® Internal Affairs does not conduct any investigations into violations of department policies or procedures. For such matters, individuals are encouraged to directly contact the respective departmental Internal Affairs Office.
All information submitted is strictly confidential. Disseminating any information outside this form is considered Influencing an Investigation as outlined in Rules & Regulations which is punishable up to termination.
