Each dancer over the age of 17 must complete the form. For dancers under the age of 18, this form must be completed by a parent or guardian. Please note: you should ask all living cancer warriors for their permission to submit their name and type of cancer. En Pointe for Ava is honored to recognize any warrior battling any type of cancer. If you are dancing for Ava, En Pointe for Ava grants permission for Ava Handley's name to be used. She battled brain cancer called DIPG.

I acknowledge that En Pointe for Ava will not be responsible for the care and safety of children at The Grand Ritz Theater at any time on Sunday, May 26, 2024 which includes but not limited to: during rehearsal, during the break between rehearsal and performance, during the performance, and/or after the performance. Children under the age of 14 must be supervised by a parent, designated dance parent (over the age of 18) or guardian at all times. If children haven't been collected by the parent after the event, En Pointe for Ava will not be held responsible, will not remain at the theater and will notify the authorities. *
