Certificate of Insurance *

If I am a business or organization. I agree to provide Lincoln Christian Church with a certificate of insurance listing Lincoln Christian Church as an "additional insured." Certificate must include liability coverage, workers compensation coverage, and medical expense coverage. I understand that my event may be canceled if the correct certificate is not provided to Lincoln Christian Church at least 30 days in advance of my event.

Cancellation Policy *

I agree that fees must be paid in advance of the event. I also agree that I may cancel my event for a full refund if I call the church office 24 hours in advance of my event (or by 5:00 p.m.on Friday for events scheduled on Saturday or Sunday).
