Who should fill out this form?

The call is open for NEW MEMBERS to join our successful EDI in LTC Community of Practice (CoP)! The EDI in LTC CoP is intended for those representing LTC homes in Ontario. This includes team members and leaders who are working in LTC homes across Ontario. We also welcome engagement from representatives from associations/organizations supporting LTC homes, and members of the Ontario CLRI's EDI Advisory Committee. In these sessions, we engage with participants from a variety of backgrounds and perspectives in the LTC sector to enrich the experiences shared during CoP sessions. Applications are welcomed for those who are at any stage of their EDI journey. Applicants must be willing to share thoughts and experiences during the CoP sessions and be prepared to apply what they have learned to their practices/organizations.

NOTE: If you have already been participating in the EDI in LTC CoP sessions, there is no need to fill out this form - this is for NEW MEMBERS ONLY.

This Community of Practice has explored a number of topics, specifically tailored to the context of LTC. Some of the topics covered in previous sessions include:
  • Supporting Indigenous culture and advancing the Calls to Action from the Truth and Reconciliation Commission
  • Addressing racism
  • Supporting the 2SLGBTQI+ community
  • Sociodemographic data
  • Policy and Quality Improvement Plans (QIP) development, with an equity lens
  • EDI strategy development
  • And more (topics selected based on the needs and interests of the group!)

Please be advised that a limited number of spaces are available for this round of sessions, in order for us to support meaningful engagement in smaller groups for breakout rooms. Should the CoP be oversubscribed, we will review all submissions to maximize equitable distribution of LTC homes from across the province.
