Liability Waiver

Student, student’s parent, or legal guardian [heretofore referred to as the CLIENT], hereby grants permission for the student to participate in classes, rehearsals, performances and other events held by Pas de Deux Dance Studio. The CLIENT has read the studio guidelines and dress code and understands that Pas de Deux Dance Studio reserves the right to deny admission, at its sole discretion, to any class, performance or event for failure to comply with these guidelines.

Participation and Medical Emergencies

The CLIENT understands and agrees that by participating in any dance class, rehearsal or performance (including competition), there is a possibility of physical injury or illness, and therefore agrees to assume all risks of any such injury or illness which might occur during any and all of Pas de Deux Dance Studio’s classes and activities. The CLIENT further grants permission for Pas de Deux Dance Studio, its owner, directors, employees, agents, volunteers and assistants [heretofore referred to as PAS DE DEUX] to procure any necessary medical help for the student while he/she is under their supervision and authorizes PAS DE DEUX to take all reasonable measures to treat any injury or illness that the student may suffer while participating in any Pas de Deux Dance Studio activities.

Release

By signing this form, the CLIENT hereby forever releases and discharges PAS DE DEUX from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury or illness incurred by the student, student’s guests, or either’s personal property which may arise out of, or in connection with, participation in any classes or activities conducted by, or in association with, Pas de Deux Dance Studio (including but not limited to performances, competitions, trips and special events), whether such loss, damage, injury or illness results from negligence of PAS DE DEUX or from some other cause.

Illness Acknowledgement of Risk

An individual can be infected with germs (such as covid-19) without knowing and be asymptomatic; a
person may feel well and spread those germs. Any CLIENT choosing to enter Pas de Deux Dance Studio or attend an event in conjunction with Pas de Deux Dance Studio, assumes personal responsibility for the risks of being exposed to germs and any related symptoms, illness or infection that occurs as a result. By entering the studio or attending an event, the CLIENT also agrees to abide by all mitigation efforts in place at the venue, and understands that these guidelines may change at any time as the situation warrants.
