ASCS Mass Registration Form
This form must be completed by all ASCS School Mass Attendees over the age of 12 by noon the business day before the scheduled Mass day. 

Attendees will be checked off by ASCS staff before entering.

There will be no entry without your name on the list.
Doors will be locked at 9:55am so that our staff can attend Mass.  No entry after 9:55am.
