Application procedures

Complete the application form, together with an $6000 payment. Please follow the "payment method and procedure" and attach your deposit statement to info@hkpcca.com or Whatsapp (67933797). If you choose to pay by a crossed cheque payable to “Hong Kong Professional Counseling & Consulting Association Ltd.”, please send it to: HKPCCA, Suite 1107 Two Chinachem Exchange Square, 338 King’s Road, North Point, Hong Kong (Attn: Ms Tiffany Li).
Our play therapist will contact you for parent consultation.
PCCA will refund the application fee when the service is not available.
** Special features for senior therapist service**: PCCA will send the child a book “The first 6th year of child” OR The second 6th year of child” as gift (dependent on the child's age) and free parenting tips. The gift is subject to a first-come-first-served basis while stocks last.
** Special features for senior therapist service**: after termination of play therapy service, the therapist would schedule a free zoom/phone consultation with the parent to follow up with the child's situation.
If more than one child needs the service, please apply for the service of the next child after the service for the existing child is finished.

請填妥此表格及繳交費用$6000。請參考以下付款方式及程序，完成後，請將付款證明電郵至 info@hkpcca.com，或Whatsapp (67933797) 本會作為憑證。如選擇支票方式付款，可將表格及支票郵寄至: 香港北角英皇道 338 號華懋交易廣場二期 1107 室(Ms. Tiffany Li)。
本會收到表格後，遊戲治療師會聯絡申請者，並安排家長諮詢。
如申請者不適合參與此服務，本會將退回全部申請費用。
**資深治療師服務**：根據孩子的年齡，本會會送出「孩子的第一個六年」或「孩子的第二個六年」一書，以及親子溝通tips作為禮品，送完即止。
**資深治療師服務**：在孩子完成遊戲治療服務後，治療師會在一個月後與家長預約一節免費30分鐘zoom/電話諮詢以跟進小朋友情況。
如家中有多於一位孩子需要服務，請與您的治療師商量，並等待HKPCCA的回覆。
