Registration form to enroll your dancer(s) and secure a spot in classes.
Completion of and payment for this form reserves a spot in the classes selected for the named dancer(s) through the end of the season in June.
A non-refundable fee of $30.00 per dancer (discounted during open pre-registration period for existing MCSD dancers) is due upon submission to complete registration process.
Once the office receives your completed registration, an account will be established and an invoice will be issued to your email on file. You may pay the invoice online via Credit Card, Debit Card, Cash App Pay, or ACH Bank Transfer. If you would prefer to pay in person, please stop into the studio during operating hours where you can pay with Cash or Check, as well as the previously mentioned payment methods.
