Live Oak Public Libraries offers proctoring services for individuals engaged in distance learning, assessments and certifications by appointment only. Both paper-based and computer-based exams can be proctored.
Library staff must have access to all test materials, physically or electronically, before being able to proctor the exam. It is therefore required that individuals requesting a proctor complete this form to make their appointment to request availability and discuss requirements for the exam.
Proctoring Services are $25 per exam (non-refundable) and may be paid with cash, credit card, or check (made payable to Live Oak Public Libraries).
Live Oak Public Libraries will be responsible to:
● Correspond with and receive test materials (email, fax, and mail)
● Distribute test materials
● Provide a place to take the exam
● Return pre-stamped, postage paid, and addressed envelopes as directed by examining institution
● Sign appropriate paper work
● Verify test taker’s identification
Live Oak Public Libraries will not be responsible to:
● Contact test takers to schedule test or make test arrangements
● Cover printing, faxing, and/or postage fees associated with the test
● Ensure or locate exams lost by email, fax, or mail
● Provide answers to test questions
● Provide proctor’s personal information
● Provide supplies needed for test
● Provide constant one-on-one monitoring
● Reschedule missed exam times
● Resolve technical Issues
