2017-18 ECC Company COMMITMENT CONTRACT AND CODE OF ETHICS FORM
As a member of the Evergreen Children's Chorale (hereafter referred to as ECC), I hereby commit to abide by the following expectations.

1. To pay my annual membership fee of $675 payable to: Evergreen Children's Chorale. $50 of this fee is non-refundable.  A written request for a refund must be given by the 9/16/17.  You may also choose to pay by semester, participate in the fall only, or participate in the spring only.  If so, dues are $325 for fall and $450 for spring.  I understand that after the due date, I will not be allowed to rehearse and will be counted absent, until my dues are paid.  

2. I understand, as an Evergreen Children's Chorale member, that my first performance commitment will be to the activities of the Chorale.  I further realize that I am an important member of the ECC; I was selected because of my talent, enthusiasm, and dedication.  Thus, I agree that if I am asked to, or personally wish to try-out or participate in a production outside the Chorale, I will show courtesy to my fellow Chorale members by discussing the dates and responsibilities involved with the Director before I make such a commitment.

3. To attend all rehearsals and performances of the ECC in accordance with the schedule, not missing more than three rehearsals per semester.  An absence should be reported to Executive Director Justin Johnson prior to rehearsal time via text or email. I will make a commitment to be at all tech week rehearsals in the last two weeks before the show.  Learning music and choreography missed due to absence will be my responsibility prior to the next rehearsal.    I will be on time for arrival to and departure from all rehearsals, performances and special events. If I am going to leave early, I will get permission from a staff member first.

4. To abide by all the rules, responsibilities, and expectations as outlined in the ECC Member Handbook.  

5. To help my parents, when possible, participate in the responsibilities and duties of, at least, two support/volunteer committees for the activities of ECC.

6. To support and actively participate in any fund raising events for the benefit of ECC.

7. To be responsible for the uniforms or costumes necessary to the production of the bi-annual programs.  Boys are expected to purchase black, cotton, double pleated pants for the fall show.  Both boys and girls are expected to provide leather ballet shoes for the fall and spring.  The official CHORALE uniforms are provided by the ECC and are its property. Most costumes will be provided by the costume committee, although at times we may ask for additional help (i.e. tights, shoes, misc.).

8. To conduct myself at all times in a manner consistent with the honor and privilege associated with membership in this organization and abide by the rules and expectations set forth by its governing Board and Directors.

9. To return all required signatures and forms by the fall semester tuition due date.  I understand that if I fail to return the necessary signed forms by the due date, I will not be allowed to rehearse and I will be counted absent, until my forms are turned in.

I have read the I have read the Handbook and understand the rules and responsibilities that my child and I will be expected to uphold as a member of Evergreen Children's Chorale.
