2018 FMCMEA Honor Jazz Registration & Release Form
Complete and submit this form no later than Monday, April 9th. A $20 registration fee (cash or checks made out to FMCMEA) is due to your director and must be submitted to the appropriate FMCMEA Representative on or before Wednesday, April 11, 2018.

You will receive a confirmation email from Barb Shinaver of your registration and your specific audition time slot by the evening of Wednesday, April 18th.
