2023 Summer Camp & Summer Programs Application
Before making an application, please contact your summer camp or summer program of preference and ensure an opening is available.
Please read the following instructions carefully and make sure that your application is complete.
Incomplete applications will not be considered.

• Separate applications must be filled out for each child
• References should be an outside professional contact (e.g. educator - teacher, EA, principal,vice-principal,
        doctor, caseworker, social service workers...) and cannot be the person applying for the funding
• The Foundation must be able to contact the reference
• Applications must be submitted before registration for activity (usually takes about 2 weeks for approval)
• Up-to-date contact information is necessary for us to contact the applicant. If we cannot contact you, we will not consider your application
• Please provide us with any other information that may be important to this application
•      Please note – if your child is a participant in our Campership Fund Program, they are still eligible to apply
        for other CFM funds throughout the year
