CCP Counseling
Ohio law requires secondary schools to provide CCP counseling to students (grades 6–11) and their parents/guardians before participation in CCP. This form is designed as an alternative method to deliver the required counseling topics when a family cannot attend a scheduled meeting. It is critical that both students and parents/guardians read, watch and sign off on all of the material together.
This is only required to be completed for first time CCP participants prior to taking any CCP class. 
Students who have already completed CCP counseling do not need to complete it again.

  2. Watch the Academic Guidance and CCP video (6 minutes)
  3. Read the Ohio Official Counseling Form page 1&2
