Place: B.J. Hackler Park, Saint Joseph IL

Please arrive between 10:45 a.m. and 11:00 a.m. to start registration. Entering a pet (or pets) is free, but there will be a spot for free-will donations to the Champaign County Humane Society. Any contributions are greatly appreciated! Please note that it is the responsibility of the owner(s) to take care of and clean up after their own pet(s). When possible, please keep pets, specifically dogs and cats, on leashes for the safety of all the animals attended. Below is a form for registration. For group costumes, each group only needs to fill out one form. 

There are 5 costume categories and the winner of each receives a ten dollar gift card to PetSmart. The categories include:
Pet and human group costume
Pet group costume
Funniest costume
Cutest costume
Best overall costume
Who will be walking with the pet during the show?
