2. Accept consent.
3. Enter sessions required.
4. Choose payment method PayPal, cheque, bank
5.  If PayPal proceed to send form then proceed to Paypal button back on website.
6. If cheque please send with child's name on the back to address below.  Cheque made payable to K8 Coaching Limited, address K8 Coaching, 3B Yik Kwan Villa, 8 Yik Kwan Avenue, Tai Hang, Hong Kong.
7. If bank please use details below:

In enrolling in 26 coaching sessions I agree that my child will abide by the rules and regulations set out by the company. They are operated by K8 Coaching Limited on behalf of 26 Coaching. I will not hold K8 Coaching Limited responsible for any injury or death to my child or damages or loss to my property as a result of participation in these sessions.  I confirm my child is capable of swimming 200m unassisted for aquathon sessions.
