Short Term Lesson Registration

This in the registration form for Music Works short-term private lesson packages. Short term packages can be purchased at any time but do not guarantee regularly scheduled lessons with a specific instructor throughout the entire year.

Packages for short-term lesson packages are available with discounted rates compared to single lessons.  These packages expire 4 months after purchase.

A confirmation email detailing your lesson schedule will be sent from registration@musicworksnw.org. This process may take a few business days. Should your preferred dates and times be unavailable, we'll reach out with alternative options from your teacher's schedule.

Note: if you or your child are enrolled in various lesson types (such as piano, voice, etc.), a separate form is required for each individual lesson type.

Terms and Conditions
Billing: Single lessons and short-term private lessons packages of 4, 8, or 12 lessons are not billable monthly. and advance payment for all lessons or sessions is required to reserve your lesson time. Lessons expire 4 months after purchase date, and no refunds are provided.
              
Attendance and Rescheduling Policy: Regular attendance by students is expected. Students are charged for all lessons for which they are registered. 1 of 4 lessons may be rescheduled for a mutually convenient time with the instructor if 48-hour notice is provided. Single lessons and short-term package lessons are not eligible for rescheduling, refunds, or credits if the student fails to provide at least 48-hour notice of absence. Faculty members have committed this time to your student, and we appreciate your respect for their busy schedules. 
                                                                                                                                                                        
Instructor Absence Policy: If a teacher misses a lesson/session due to illness, emergency or other reason, the lesson will 1) be rescheduled, or 2) Music Works will provide a qualified, screened substitute to conduct lessons/sessions in their absence.

Student and Parent/Guardian Agreement and Permission:

1. I have read and consent to the terms and conditions stated above. 
2. I commit to paying tuition promptly and accept any financial penalties that may accrue due to my child's or my own non-payment, or for any physical or property damage caused on the premises of Music Works Northwest. 
3. I will inform the Registrar of Music Works Northwest, as well as the student's instructor, of any medical conditions, medications, or special medical concerns or needs that may affect the student's performance and/or attendance.
4. In case of a medical emergency, I authorize any representative of Music Works to obtain medical care for my child or me. I will bear all costs related to medical treatment and emergency transportation.

Photo & Video Authorization - Internal Use

I grant authorization for Music Works Northwest to use photographs, video, and audio recordings of my student for internal use, including lobby monitors, school newsletters and displays, documentation, and in-class educational review.

Photo & Video Authorization - Promotional Use

I grant authorization for Music Works Northwest to use photographs, video, and audio recordings of my student for promotional use, Music Works' website, brochures, social media channels, and print advertising.

Note: we value student safety and will not share student names alongside photos on any public websites, posts or printed materials without your consent.

Tuition Assistance

Music Works offers tuition assistance for families based on demonstrated financial need. Tuition assistance funds are limited, and a separate application is required.
