  • Athlete's CBTF number
  • Club Name
  • Coach(s) & Choreographer membership #'s (contact your coach for this information)
  • Coach membership number
• At this time, we are not able to accept multiple entries on one form. If you have more than one member in the family, you will need to complete one form for each athlete
• Age Categories for individual events:
  • Juvenile - 9-11 yrs
  • Junior - 12-14 yrs
  • Senior - 15-17 yrs
  • Collegiate - 18+ yrs
• If you accidently choose an event that your child should not be entered in, please select the 'Deselect Entry' option
