Astrology intake form
Thank you for your interest in an astrology reading with me! I so look forward to journeying with you into the heart of your natal chart. My readings are designed to be experienced as a conversation and so I encourage you to share and dialogue with me each step of the way. Typically, during a first-time reading, I'll aim to cover the most important signatures and key patterns in your natal chart. Depending upon how long that takes, we may (or may not) have time to very briefly explore the Progressed chart and transits.

Prior to a reading, I ask you to please submit this intake form in which you are invited to share pieces of your personal history and the reasons for seeking a reading. I find that this activity enriches the experience all around. In fact, I would say that this exercise is the first part of an astrology reading experience with me.

With the exception of the question about your birth data, you're free to answer in as much or as little detail as feels comfortable. If you feel called to answer the questions but also overwhelmed or stumped, my suggestion is to start exactly with where you are and with whatever is arising in that moment. You will see as you allow the words to flow, that what comes through is exactly what needs to. It's all relevant. It's all consequential. It all has meaning.

Please ensure you complete this form at least three days prior to your scheduled reading otherwise I may ask to reschedule the reading. You can book a slot here: https://calendly.com/shebaremy. Please do reach out if you can't find dates and times that work for you. To complete your booking, please send payment along with this form.
