To file a report with the Nowhere's ConsentCommittee, please fill out and submit this ***Incident*** Form. This form collects information regarding possible ***Code of Conduct violations***. The completed form should contain sufficient detail for the investigators to begin an investigation. If the report warrants an investigation, Consent Committee Leads will review the report with you before assigning your case to a team of two investigators who you will have a chance to approve of.

Include or indicate any alias. You can also give us any physical detail, you would remember if you don't know the name of the person. However it is ok if you don not remember anything, this is part of the injury you go through, nothing you should blame yourself for, right ?

For example, has the person attended Nowhere Camping festival or Nowhere Burning Pub up events and/or related burner events such as Decom, or other burn events inside or outside the local community? *

The Consent Committee (Off during the event) seeks your intention for filing this report.

Please note: For urgent issues during hiatus, you can meet the Safer Space Crew beside Welfare Enough (11am to 1pm, Tuesday to Sunday) for a private chat or go to the Malfare tent and ask theLead. You will receive an email from the Consent Committee when they return from hiatus telling you if they can open a session with the person of concern, if you agreed for further contacts.

The Consent Committee is on hiatus from June 15 till the end of September Malfare / No-Mads / Safer Space / Welfare Enough are available, 24/7 on site, to support you if necessary.

The Consent Committee will not pursue legal action, however if you were to do so on your own, all recorded communication received by the Consent Committee (starting with this report) can be subpoenaed under certain circumstances.
