Please fill out information for all volunteers. Minors will need a parent or legal guardian signature in the last question on the form. Please fill out the waiver for each minor.

This waiver does not expire.

Court ordered community service placements must be approved before registering for volunteer sessions. We do not work with volunteers whose court-ordered community service is related to violent crimes, sexual assault/sexual offender, or theft.
