Lakeland School Corporation: 2023/2024 Transportation Request Form

Please use this form to request transportation for your student(s) for the 2023/2024 school year.

In order to secure a place on a route you must submit this request by August 3, 2023. Any requests received after that date can not be guaranteed a ride on the first day of school.

All bus stop times and locations will be sent, via email and or text during the week of August 7th.

Please note the following:

1. All students may select one AM and one PM bus stop. For maximum route efficiency and student safety, we can not accommodate split schedules such as grandma's house on Monday and home on Tuesday-Friday.

2. Notes for one time bus changes for sleepovers or birthday parties are never accepted, parents should plan for alternative transportation arrangements for such events.

3. One student per submission please.
