I understand that the client testimonial for BMI of Texas will only include my name, location, photograph, and information provided to the organization in my testimonial. I understand that all other protected health information that BMI of Texas creates and maintains for purposes of my care will not be used in my testimonial or for marketing purposes without prior authorization per privacy regulations of the state and Health Insurance Portability and Accountability Act (HIPAA).
I also understand that once my images are published, I lose control and rights to these images, that the individual social media platforms may assume control and rights to those images, and that images posted on the Internet can be altered and/or archived and are permanent and searchable.
