End of Year Survey 2024-25 

WE WANT TO HEAR FROM YOU! 

Bernards Special Education Parent Advisory Council (SEPAC) is looking to better understand your family's special education journey, particularly over this past year. Through these survey questions, SEPAC will be able to identify global trends across our schools and programs so we can create our 2025-26 focal points and goals. Please take this opportunity to share what you have found successful as well as your suggestions for growth and improvement. 

PLEASE NOTE:
1. THIS FORM CAN BE COMPLETED ANONYMOUSLY.
2. The only questions that "require a response" are your child's school, class placement and current education plan so we can accurately categorize the feedback we receive.
3. While the district supports our efforts, this is not a district survey. SEPAC will directly receive all survey submissions and will NEVER link anyone to specific data. 
4. If you have multiple children that you'd like to give feedback for, PLEASE SUBMIT ONE FORM PER CHILD.

What is the Special Education Parent Advisory Council (SEPAC)?
Bernards SEPAC is a parent-led, state-mandated group focused on policies, practices and programs affecting the education, health and safety of children receiving services under an IEP or 504 plan from Bernards Township. The SEPAC Executive Board works closely with BTSD administrators and BOE to share feedback, identify trends and collaborate on improving the Special Education experience for all families.
