Use this form to alert EastMO about which student(s) you are entering in the NSDA LAST CHANCE QUALIFYING TOURNAMENT.

A reminder, to be eligible:
1) The SCHOOL must have competed in the District Tournament Series
2) A STUDENT must have NOT qualified to NSDA Nationals in any event
3) A STUDENT must meet NSDA eligibility requirements for Districts.

This is NOT the official registration - this form simply alerts EastMO so that we can plan lodging for your students!  Official registration will go through Tabroom.com.
