RCES Change in Dismissal Form
For the safety of ALL students at Rachel Carson Elementary School, this form is to be used when your child’s after-school dismissal plans change. Please complete the form and send it to school in your child’s backpack for his/her teacher. All dismissal changes must be in writing (or on this form), signed by the parent or legal guardian, and received by 3:00 p.m. Phone calls are discouraged to change dismissal plans and should only be used as a last resort.

**Photo identification is required of anyone picking up a child from school. Parents and legal guardians must give written consent for any other persons to remove their child from school even if that person is listed on the child’s emergency procedure card. Emergency contacts listed on your child’s emergency procedure card are persons you give the school permission to contact if you cannot be reached during the school day and your child needs to be  picked up in the event of an unexpected emergency such as an illness or injury.
