Accessibility Compliance Fund Request
Please fill out this form in order to request financial support from the SFDCCC in order to comply with a request for accessibly accommodation for a public meeting or event. Only current chartered clubs of the SFDCCC shall be eligible for these funds.

In order to complete your request, you must send a copy of the invoice that clearly lists amount requested and, if relevant, breaks out the specific services/equipment being provided. Invoices should be sent to Executive Director Adam Mehis at adam_mehis@sfdemocrats.org.

Funds requested may be either a reimbursement to the club for services already rendered, or payment may be made directly to the vendor providing the service/equipment.
