Countywide Patch Request Form
We have a limited supplies of patches, so orders are not guaranteed. Our patches are a first-come-first-serve basis. Please submit the request ASAP to help reserve your patches. This form is checked for Creek to Bay patches from April to May and for Coastal Cleanup Day patches from September to October. If you submit after those months you might not receive your patches.
