Standard Absence Form
If a student will be absent or late due to illness or an unplanned appointment, a parent or guardian must notify the school secretary and complete the Standard Absence Form.

Notify the school secretary by phone call or email before 8:00 AM on the day of the absence or late arrival.

Complete the online Standard Absence Form by the end of the absence day.
An absence is considered excused if it is the result of illness, quarantine, medical or dental appointments, family emergency, or death of a family member. Other reasons are unexcused.

A maximum of ten days of cumulative excused absences verified by parental/guardian notification may be permitted during a school year. All absences beyond ten cumulative days will require an excuse from a physician.

If arriving after 7:50 AM, the student must check in with the school secretary.

Without notification and the completed form, the absence is unexcused.

Each individual day of absence requires completion of the form.
