National Form: Request for Assistance Due to COVID-19 / Solicitud Nacional: Solicitud para recibir apoyo debido a las afectaciones del COVID-19
Dear Community Members:

First, thank you for your courage. Asking for help takes strength and it's one bravest things you can ever do!

If your livelihood has been affected and greatly diminished by the COVID-19 pandemic, if you are an hourly wage worker, rely on tips, work in hospitality or events, or anything else - please use this form to let us know what specific ways our community can come together to help you in your time of need.

We will partner you up with a family or individual that can help you meet this need.

Please note this is a MANUAL process and there are hundreds of volunteers working around the clock to generate the email that will connect you with a patron, so you may not hear from us for up to 5 days from the time you fill out this form. In some communities this time frame may be more and in some less - it really depends on the volume of patrons willing to assist in your area.

Some of you will have more than one patron assigned so you will get MORE than one email. We are trying to make you as whole as possible in the coming month(s) ahead.

This form will remain confidential and we will NOT share your personal information with anyone until you are matched up.

This is our time to come together as a community to support each other and to create an outbreak of LOVE over FEAR and we are so happy that you chose us to assist you in some way. It is an honor.

Si tus ingresos se han visto afectado por la pandemia del COVID-19, si trabajas por horas, o tu trabajo es en el ámbito de la hostelería, eventos, o circunstancia similar - por favor usa esta solicitud para hacernos saber de manera específica te podemos apoyar como comunidad en estos momentos difíciles.

Te conectaremos una la persona o familia para que te brinde el apoyo que solicitas.

Ten en cuenta que este es un proceso MANUAL con voluntarios trabajando para realizar las conexiones y generar el email que te conectara con tu “patron” (persona o familia que te ofrece el apoyo que solicitas), este proceso puede tardar hasta 5 días. En algunas comunidades el tiempo puede ser mayor y en otras menor - depende de la disponibilidad de “patrons” en tu área.

Algunas de las personas que solicitan apoyo serán conectadas con más de un “patron” para lograr cubrir todo lo que solicitan, por lo que recibirán más de un email. Estamos trabajando para cubrir todas las necesidades posibles que puedas tener durante los próximos meses.

Tu información se mantendrá confidencial, NO compartiremos tu información personal con nadie, solo pondremos tu teléfono e email en el email de conexión.

Son tiempos de unirnos en comunidad y apoyarnos mutuamente creando una pandemia de AMOR en vez de miedo, y estamos muy felices de que nos escogieran para asistirte, es para nosotros un honor.
