Ice Skating Parent Form/Waiver
Dear WHRHS Parent/Guardian,

Your student has RSVP'd to attend the free PTO-sponsored Ice Skating Student Social.  

There are 2 more things needed FROM YOU to make this night safe and successful.

1) EVERY STUDENT ATTENDING must complete the Bridgewater Arena waiver attached in Section 2 of this google form. 

If the student is 18 or over, they can fill the waiver out themselves, otherwise a parent must sign it and the student can hand it in the night of the event. 

2) One parent/guardian must acknowledge the Code of Conduct listed below. 
If the student is 18 or older, the parent does not need to complete this section.

Thanks to the generosity of the WHRHS Community, the PTO is able to host this exclusive event FREE OF CHARGE for current WHRHS Students.  

This event is an EXCLUSIVE & PRIVATE event for WHRHS Students.  Our Warriors will have their own time on the ice PLUS a private DJ to add to the fun!  Skate rental Included!

Date:                   Saturday, March 4, 2023 

Time:                  8:15pm-10:15pm 
                            Arrive by 7:30pm for skate rental (skate rental included)

                            Bridgewater, NJ 08807

Food & Drink are available for purchase at the Bridgewater Sports Arena Cafe. Cash/Credit Cards only. 
No Apple Pay available. No outside food or drink allowed.

REMEMBER TO DRESS WARM, WEAR A COAT, GLOVES AND LONG SOCKS!

While this is not a school event, the same conduct and rules are expected including:

-  No outside food or drink, large bags, purses or backpacks. 

-  Students may not leave and return unless escorted by a chaperone, providing one is available.

-  No one will be permitted if under the influence of drugs or alcohol. 

-  Parents/Students are responsible for transportation to and from the event.
